A National Training System for Simulation in Anaesthesia & Intensive Care and other Specialities

Anaesthesia & Intensive Care in adult and paediatric care is characterized by a high complexity of manoeuvres and specific procedures, as well as very complex clinical scenarios. There is an obvious need for stronger skills- based training as required by the European training models. In order to develop and maintain high training standards, Romania has recently introduced simulation methodology into continuous medical education (CME) at University level. However, the training capacity in in Romania remains limited: from the leading five universities, only two (Cluj, Tg-Mures) have simulation centres which are to some degree functional.

Background

One main goal of the TF Health is to improve quality of health services through strengthening professional capacities of medical staff. The Ministry of Health of Romania (MoH) currently develops a training program (Atestat) for paediatric intensive care and implements a related training of trainers (ToT) program. Another TF Health project develops and updates curricula for emergency care. Simulation training is a priority for the national training system and the MoH has identified simulation training in Anaesthesia and Intensive Care as the way to complementary continue the above TF Health projects towards an increased training quality in line with the European standards. Moreover, the simulation training centres will offer an ample and up-to-date educational practical support to young doctors and nurses in intensive care.

Based on the project outline of the MoH, the Steering Committee has endorsed the proposal in January 2016 and the OPZ-NMS in February 2016. Based on the needs assessment, the project document has been elaborated by the MoH and Swiss Intermediate Body (TPH), in intensive and time-consuming consultation with the SCO and SDC HQ. This consultation has allowed to insert the good practices from other SDC projects (e.g. Kirgizstan / Tajikistan / Moldova / Ukraine) and to improve the project document substantially, in particular with regards to sustainability aspects and accreditation) and based on the OPZ recommendations.

Objectives Intensive care patients benefit from improved quality of critical care and safe procedures offered by well-trained physician in the Simulation Training Centres.
Activities The needs assessment reported necessary information for the implementation including: list of simulation equipment to be procured and technical descriptions, recommendations for training sites rehabilitations, draft list of simulation scenarios, estimated budget and other. The training sites will be rehabilitated by the Universities and the MoH will procure and assure the installation of the simulation equipment in the rehabilitated training sites. Meantime the MoH and Swiss Intermediate Body (SIB) will contract an executing agency to implement training activities such as: develop accredited curricula and simulation based training programs, train the trainers, develop together with the Universities their business strategies to assure sustainability of the training programs, test the simulation training programs and assess its functionality.
Results

Expected results:  

  • Five university centres have created the infrastructure necessary to perform simulation training activities in anaesthesia and intensive care, intensive child care and other specialties.
  • The centres’ organisational charts include multidisciplinary teams having the ability to carry out simulation training activities pursuant to the curriculum.
  • Five university centres are provided with equipment which will cover the training needs in anaesthesia and intensive care, intensive child care and other specialties.
  • Integrated Curriculum for simulation training in anaesthesia and intensive care, paediatric intensive child, developed and accredited pursuant to further medical education standards.
  • The five universities have accredited simulation training programmes in anaesthesia and intensive care, including intensive child care.
  • Regular Supervision and assessment of performance, based on specific questionnaires and a structured feedback. The five universities have specific assessment questionnaires.
  • Development of business strategies which will ensure medium- and long-term sustainability to the centres.
