U.S. Social Security Card - First time application

Required documents for applicants who have never been issued a social security number

Applicants born in the U.S. or Iran:

  • Original valid U.S. passport of the applicant (if the applicant is below 16 years old, the valid U.S. passports of the U.S. citizen parent(s) is also required.
  • Original U.S. Birth Certificate or CRBA (Consular Report of Birth Abroad) or Naturalization Certificate of the applicant.
  • Form SS-5 "Application for a Social Security Card"
  • Form ''Sworn Affidavit'' (only available at our office).
  • Form 1706-f3 for U.S. born applicants only. (Available at our office).
  • If the applicant is above 12 years old, the entire original school transcripts of the applicant, starting from elementary school to present, officially translated in English and certified by the Iranian Ministry of Foreign Affairs is required. 
  • No shool transcripts are required for applicants below 12 years old.

Please note

The personal presence of the applicant (s) is mandatory, minors have to be accompanied by at least one parent

Applicants can submit the mentioned documents, by appointment only. Appointments can be scheduled by phone or physical presence

Only complete applications can be accepted and further processed without any exceptions. Incomplete files will be refused and are subject to new appointment.

Requested forms should be duly signed and dated during your visit at this office.
